There are two distinct applications you may need. The Registration Application is for individuals who do not yet have a status number. The SCIS Application is for citizens who already have a Huu-ay-aht First Nation status number and are applying for a status card.
Within the Huu-ay-aht First Nation, there are two designated IRAs, a primary and a backup, who are available to assist all citizens in ensuring that their applications are completed accurately and with the proper documentation. The primary IRA is Jordan Young, Registrar Clerk (citizenship@huuayaht.org).
For Registration of children 15 or younger or dependent adults, you’ll need
- Long form birth certificate
- Photocopy of any legal documents pertaining to the custody or guardianship of the minor, if applicable
- Original name linking documents, if applicable
- 1 valid piece of ID from 1 parent (needs full name, date of birth, photo and signature)
OR 2 valid pieces of ID that combined have full name, date of birth, photo and signature
OR 1 valid piece of ID that is missing one of the requirements and a guarantor
- Photocopy of the adoption order, if applicable
- 2 identical and unaltered passport-style photos, taken within the last 12 months, if requesting a SCIS card along with the registration
- A signature from both parents/legal guardians
Application Form for Children 15 or younger or dependent adults
For registration of children 16 or older or adults, you’ll need
- Long-form birth certificate
- Original name linking documents, if applicable
- 1 valid piece of ID from the applying parent (needs full name, date of birth, photo and signature)
OR 2 valid pieces of ID that combined have full name, date of birth, photo and signature
OR 1 valid piece of ID that is missing one of the requirements and a guarantor
- Photocopy of the adoption order (if the applicant was adopted as a child), if applicable
- 2 identical and unaltered passport-style photos, taken within the last 12 months, if requesting a SCIS card along with the registration
Application Form for children 16 and older or adults
For SCIS (5-year status card) applications for children 15 or younger or dependent adults, you’ll need
- Photocopy of any legal documents pertaining to the custody or guardianship of the minor, if applicable
- Original name linking documents for the applicant or parent/guardian, if applicable
- 1 valid piece of ID from 1 parent (needs full name, date of birth, photo and signature)
OR 2 valid pieces of ID that combined have full name, date of birth, photo and signature
OR 1 valid piece of ID that is missing one of the requirements and a guarantor
SCIS Application for children 15 or younger or dependent adult
For SCIS (10-year status card) applications for children 16 or older or adults, you’ll need
- 1 valid piece of ID that has all 4: Full Name, Date of Birth, Photo and Signature
OR multiple valid pieces of ID that combined have Full Name, Date of Birth, Photo and Signature
OR 1 valid piece of ID that is missing one of the requirements and a guarantor with valid ID
- Original name linking documents for the applicant
OR a copy of the name-linking document and a copy of government-issued ID with that name, if applicable
SCIS Application for 16 and older or adults
Consent to transfer
If you have status with another nation and are transferring to Huu-ay-aht First Nations, you will need to submit a consent to transfer with your citizenship application to receive a Huu-ay-aht status number.

Guarantor Declaration
A guarantor declaration is needed any time that there is an application being filled out without the assistance of Huu-ay-aht First Nations IRA.
If a guarantor is needed, they will
- need to have status
OR an occupation listed on the declaration
- need to fill out and sign the Guarantor form
- need to fill out a portion in the SCIS Photo App including signature
OR Write the name of who the application is for, sign, and date on the back of 1 of the 2 passport-style photos being submitted
